1. Please Upload the Following Documents

Please secure and upload digital copies of:

  • Signed Service Agreement (Anvil Email After Dashboard Creation)

  • DD214s

  • Service Treatment Records

  • Any private/civilian healthcare records

  • VA Healthcare Blue Button Report

  • Any additional outside provider records (Urgent Care, Physical Therapy, Dermatology, Cardiology, etc.)

A. How to Download Your VA Healthcare Blue Button Report

  1. Log in to VA.gov

  2. Access MyHealtheVet from the top right tab

  3. Select “Get medical records”

  4. Scroll down and select “Download your medical records”

  5. Select “Select records and Download report”

  6. Under Date Range, choose Custom, then select from your date of separation/retirement to today

  7. Check the box “Select all VA records” and select "All time"

  8. Select PDF format

  9. Download the file to your desktop

  10. Upload the file into the Evidence folder in your Dashboard

B. How to Download Your VA Claim Decision Letters

  1. Log into VA.gov

  2. Go to MyVA

  3. Select Manage all claims and appeals

  4. Under this page choose “Your Claim Letters”

  5. Select “Download your VA claim letters”

  6. Scroll down to the dates following your separation/retirement

  7. Download each letter that contains (Claim Decision )

  8. Upload all files into the Evidence folder in your Dashboard

C. How to Retrieve Private Treatment Records

  • Call your Primary Care or Specialist Provider and request they send digital copies of your recorded visits. 

    • Additionally, it would be prudent to ask your provider if they would be willing to write a small note in regard to your treatment which assist in our claims process. 

    • These records include visits pertaining to common flu or cold symptoms, headaches, treatment for pain or 

D. Final Step

  • If you Service Treatment Records are missing or unavailable, we ask that you use the following link to request a Freedom of Information Act for copies. This process can take upwards of 9mo to complete, however DOES NOT halt the claims process. It is most prudent for us to see them, but not mandatory for submission.
